1. Untersuchungen über die Pathogenese von Harnsäuresteinen müssen die Art des zugrundeliegenden Leidens berücksichtigen. 2. Bei vermchrter Harnsäureausscheidung (z.T. Leukämien) ist die hohe Harn-säurekonzentration im Endharn Ursache der Steinbildung. 3. Bei familiärer Hyperurikämie bzw. Gicht ist die Harnsäureausscheidung meist normal. Hier wird der Defekt einer distalen Harnsäuresekretion angenommen mit verminderter Harnsäurerückresorption im proximalen Tubulus und erhöhter Harnsäure-konzentration im Bereich der ersten Hamkonzentrierung. 4. Auch wenn keine Grundkrankheiten nachweisbar sind, ist es zweckmäβig, zwischen hyperurikämischer und hyperurikurischer Steinbildung zu unterscheiden.
