Bei sechs niereninsuffizienten, dialysepflichtigen Kindern und Jugendlichen im Alter von acht bis 21 Jahren wurden die Auswirkungen einer L-Carnitinsupplementierung in einer Dosierung von 10 mg/kg/d und nachfolgend 100 mg/kg/d auf Parameter des Lipidstoffwechsels untersucht. Die L-Carnitinsubstitution erfolgte über vier Wochen pro Dosierungsregime. Darunter kam es zu einem signifikanten Anstieg der Serumcarnitinkonzentration bei gleichzeitigem Abfall des Acylcarnitin/Freies Carnitin-Quotienten von x: 1,59 auf x: 0,83. Im gleichen Zeitraum erfolgte ein signifikanter Abfall der Serumtriglyceridkonzentration sowie ein Anstieg des Serum-HDL-Cholesterins. Zwischen HDL-Cholesterin und dem AC/FC-Quotienten konnte eine inverse Korrelation (r = ––0,63; p < 0,0008) nachgewiesen werden. Die L-Carnitinsupplementierung von niereninsuffizienten Patienten kann durch die HDL-Cholesterinanhebung als antiatherogene Maßnahme angesehen werden.
