Das Antiarrhythmikum Amiodaron (Cordarex) ist eine der häufigen Ursachen schwerer, jodinduzierter Hyperthyreosen. Aufgrund der mehrmonatigen Halbwertszeit des Medikamentes und der vollen Jodspeicher sind thyreostatische Therapien wenig effektiv, so dass eine notfallmässige Ablation des Schilddrüsengewebes erforderlich werden kann. Die kardiale Vorschädigung mit Amiodaron behandelter Patienten erhöht das Operationsrisiko deutlich, so dass eine frühzeitige Thyreoidektomie angestrebt werden sollte. Die niedrigen Komplikationsraten elektiv ausgeführter Schilddrüsenresektionen lassen uns eine prophylaktische Operation fordern, sowie das TSH basal unter Amiodarontherapie supprimiert und T3 erhöht sind.
