Neben der individuellen Disposition ist der Einfluss exogener Noxen für die Entstehung von Karzinomen der Luft- und oberen Speisewege weitgehend unbestritten. Eine nachhaltig intensive Einwirkung derartiger Karzinogene birgt das Risiko der lokalen Tumorentwicklung. In bezug auf die Folgen des Zigarettenrauchens hat sich bereits der Begriff «Rauchstrasse» etabliert. Voruntersuchungen zur betroffenen Seite (Links-Rechts-Verteilung) bei Einfachtumoren hatten eine diagonale Rauchstrasse nahegelegt. Unsere Untersuchung der Seitenverteilung von 126 Tumoren bei 63 Patienten zeigt nun, dass innerhalb eines Individuums synchron oder metachron auftretende Doppel-Karzinome → bei alleinigem Befall der Mundhöhle teils links und rechts, teils auf der gleichen Seite lokalisiert sind, → Tumoren der Mundhöhle überwiegend mit Rachen- oder Kehlkopftumoren der Gegenseite kombiniert sind (25:12) und → Doppel-Tumoren jenseits des Isthmus faucium (in Rachen und Kehlkopf) jeweils auf der gleichen Seite liegen (21:0). Als Konsequenz für die Betreuung unserer Tumorpatienten ergibt sich nach wie vor die Forderung nach einer Zweittumorsuche sowohl im prätherapeutischen Staging als auch während der Tumornachsorge. Bei der Panendoskopie dürfte es Sinn machen, den hier beschriebenen typischen Lokalisationen besonderes Augenmerk zu widmen.
