Adenoidzystische Karzinome (AZK) der Kopf-Hals-Region sind hÄufiger in den kleinen SpeicheldrÜsen, insbesondere am Gaumen (25%) und den NasennebenhÖhlen (11%) lokalisiert. Die geringe Symptomatik, das langsame Tumorwachstum und die Infiltration entlang des Perineuriums oder der KnochenkanÄlchen erklÄren das hÄufig bereits bei der Erstdiagnose fortgeschrittene Tumorstadium. Die Art dieses Tumorwachstums fÜhrt zu einer hohen Rate von Lokalrezidiven (80%), sowie hÄmatogenen (30%), seltener lymphogenen Metastasen (5%). Diese Wachstumskinetik bestimmt den oft langen klinischen Verlauf und die schlechte Prognose der AZK. Seit 1979 wurden an der HNO-Klinik Innsbruck insgesamt 17 Patienten mit AZK unterschiedlicher Lokalisation behandelt. Bei einer medianen Nachbeobachtungszeit von 6 Jahren werden unsere Ergebnisse nach operativem sowie strahlentherapeutischem Vorgehen dargelegt. Drei Patienten werden vorgestellt, um auf den typischen klinischen Verlauf des AZK hinzuweisen.
