Es wird über einen 44jährigen Patienten mit einem Kappa-Leichtket-tenplasmozytom berichtet, welcher bereits seit 12 Jahren wegen einer Osteoporose mit multiplen Frakturen in Behandlung stand. Zum Zeitpunkt der Diagnose bestand eine schnell progrediente Myelom-niere sowie eine ausgedehnte tumoröse Raumforderung im Beckenbe-reich. Die Polychemotherapie nach dem VMCP-Schema führte zu einer Remission. Unsere differentialdiagnostischen Erwägungen lassen uns die langjährige Osteoporose im vorliegenden Fall am ehesten als außergewöhnlich langfristiges, monosymptomatisches Vorstadium eines multiplen Myeloms und weniger als unabhängige Ersterkrankung interpretieren.
