Die transanale Tumorektomie ist eine kontinenzerhaltende Operation. Bei striktem Einhalten gewisser Indikationsparameter ist sie auch kurativ gerechtfertigt, nicht nur palliativ. Durch eine mit einer mäßig-gradigen Sphinkterdehnung verbundene laterale Sphinkterotomie der Mm. sphincter internus und externus bis auf minestens zwei Drittel ihrer Höhe können Tumoren bis auf 12 cm Höhe im Rektum von der Linea anocutanea an entfernt werden. Das Krankengut aus der I. Umversitätsklinik für Chirurgie Innsbruck vom Januar 1975 bis März 1979 zeigt, daß bei über 50 % aller transanalen Tumorektomien diese Ope-rationswinkelerweiterung notwendig war, um die Geschwülste im Ge-sunden entfernen zu können.
