Bei einem jungen, kardial gesunden Mann mit metastasierendem Pankreaskarzinom wurde eine 5-Fluorouracil (5-FU)-Dauerinfusion (25 mg/kg Körpergewicht/24 h über 5 Tage) durchgeführt. Im ersten Therapiezyklus traten nach ca. 56 h (im zweiten Zyklus nach ca. 36 h) heftige thorakale Schmerzen auf, die mit Nitraten nicht, mit Opioiden zwar deutlich gebessert werden konnten, aber erst mit Beendigung der 5-FU-Infusion sistierten. Während der Schmerzperioden wurden un-auffällige Herzstromkurven abgeleitet, die Kreatin-Kinase war nicht pathologisch verändert. Zu einem späteren Zeitpunkt war schließlich im EKG eine Narbe im Hinterwandbereich nachweisbar. Bei erneuter Infusion traten ähnliche Probleme in geringerer Intensität auf. In der Folgezeit rasche Progredienz des Tumorleidens, an dessen Folgen der Patient verstarb. Im Rahmen der Obduktion wurden zwei Myokardinfarkte nachgewiesen, eine Stenose in den Koronargefäßen bestand nicht. Als Ursache für diese Nebenwirkungen werden Spasmen der Koronargefäße diskutiert.
