Der Fall eines 60jährigen Patienten mit einer Peritonealkarzinomatose eines kleinzelligen Tumors wird vorgestellt. Initial bestand ein unklarer rektaler Palpationsbefund, welcher bioptisch ein kleinzelliges Karzinom ergab. Eine Laparotomie ergab eine groteske Peritonealkarzinomatose mit Tumormassen retro- und infraperitoneal sowie im kleinen Becken, welche histologisch der Rektumbiopsie entsprach. Die klinische und histologische Diagnose lautete auf wahrscheinliches epitheliales peritoneales Mesotheliom. Der Patient verstarb ein halbes Jahr später. Bei der Autopsie fand sich ein kleinzelliges Prostatakarzinom, positiv für das Prostata-spezifische Antigen und die saure Phosphatase. Überlegungen zur Diagnostik und Differentialdiagnostik werden dargelegt.
