Les reins de 62 sujets morts au terme d’une insuffisance rénale chronique ont été examinés à la recherche de tophus. Dans 17 cas, des tophus avec ou sans précipitations cristallines intra-tubulaires ont été constatés, dans la médullaire exclusivement. Les tophus sont faits d’une nécrose centrale, entourée d’une réaction cellulaire, et limités par une coque scléreuse. Ces lésions sont aisément identifiables à I’examen de pièces fixées dans le formol à 10%. Après fixation alcoolique, on constate que la nécrose est riche en cristaux ayant tous les caractères des cristaux en aiguilles d’urates. Ces tophus sont identiques à ceux observés au cours de la goutte primitive et des autres hyperuricémies. Ils jouent peut-être un rôle aggravant dans I’évolutivité des néphropathies chroniques, queue que soit I’étiologie. Comme ils sont sans doutes secondaires à I’hyperuricémie de I’ insuffisance rénale chronique, il semble opportun de ramener le taux de I’acide urique plasmatique des urémiques à la normale.
