Ziel dieser Untersuchung war es, die eigene Lernkurve der Sentinel-Lymphonodektomie mit den Erfahrungen anderer Autoren zu vergleichen. Resultate: Zwischen Juli 1999 und Mai 2004 haben die Autoren 218 Sentinel-Lymphonodektomien, davon 48 in der 24-monatigen Evaluationsphase mit zusätzlicher vollständiger axillärer Lymphonodektomie, durchgeführt. Die Detektionsrate betrug insgesamt 212/218 oder 97,2%, während sie in der Evaluationsphase 38/48 oder 89,6% betrug (Versager waren die Fallnummern 6, 7, 22, 24 und 47). Unter den 48 Fällen in der Evaluationsphase wiesen 24 positive axilläre Lymphknoten auf. Bei 3 dieser Fälle konnte kein Sentinel-Lymphknoten gefunden werden, und bei weiteren 4 Fällen war er im Schnellschnitt falsch negativ. Die falsch negativen Ergebnisse in der Lernphase waren die Fallnummern 2, 10, 23 und 29 (4/21 oder 19%). Ab September 2001 bis Mai 2004 betrug die Detektionsrate über 99% (165/166). Diskussion: Die eigene Erfahrung zeigt, dass die Anfangsphase mit Schwierigkeiten und Unsicherheiten verbunden ist. Die allgemeinen Empfehlungen, während der Lernphase auf die Erfahrung anderer zurückzugreifen und erst nach absolvierter Lernkurve eine alleinige Sentinel-Lymphonodektomie ohne komplettierende Axilladissektion anzubieten, dienen der diagnostischen Sicherheit und können deshalb auch anhand der eigenen Erfahrungen nur unterstützt werden.
