Wir präsentieren den Fall einer nahezu asymptomatischen, gedeckten Uterusruptur nach zwei vaginalen Entbindungen im Zustand nach Sectio bei der ersten Entbindung. Eine Narbenruptur ist auch im Status nach Sectio ein seltenes Ereignis, was den vaginalen Geburtsversuch im Zustand nach Kaiserschnitt trotz des erhöhten Rupturrisikos rechtfertigt. Besondere Sicherheitsvorkehrungen bei der Geburtsleitung im Zustand nach Sectio gilt es jedoch zu beachten. Diese wurden von der ACOG und der AG für fetomaternale Medizin publiziert. In unserem Fall erfolgte die Diagnose einer Uterusruptur aufgrund einer vaginalsonographischen Untersuchung 7,5 Wochen nach unauffälligem Spontanpartus. Eine weiterführende Untersuchung mittels MRT erbrachte keinen wesentlichen zusätzlichen Informationsgewinn, weshalb sie nur in Ausnahmefällen indiziert sein wird. Ob eine routinemässige vaginalsonographische Untersuchung nach unauffälliger vaginaler Entbindung bei einem Kaiserschnitt in der Vorgeschichte zum Ausschluss einer Narbendehiszenz indiziert ist, sollte durch weitere Studien geklärt werden.
