Hintergrund: Wie jede diagnostische Fortentwicklung in der Medizin weckte auch die Einführung der hochauflösenden geburtshilflichen Sonographie rasch positive Erwartungen. Derart rascher Fortschritt beschwört allerdings besonders in der Geburtshilfe auch eine Anspruchshaltung herauf. Fragestellung: Es stellt sich die Frage, ob und in welcher Weise die geburtshilfliche Sonographie Einfluss auf den Schwangerschaftsausgang von Mutter und/oder Kind hat, und wie ein effektives Ultraschallüberwachungskonzept beschaffen sein sollte. Material und Methodik: Die zum Teil uneinheitliche internationale Datenlage soll zu den massgeblichen Fragen der sonographischen Pränataldiagnostik analysiert werden. Ergebnisse und Schlussfolgerung: Der Nutzen vorgeburtlichen Ultraschalls in der Schwangerschaftsüberwachung lässt sich unter Berücksichtigung der medizinischen aber auch der ökonomischen Argumente eindeutig belegen. Die Auswirkungen von gesetzlichen Regularien und/oder von Empfehlungen einzelner Fachgesellschaften lassen den forensischen Hintergrund, vor dem vorgeburtliche Diagnostik heutzutage erfolgt, deutlich werden.
