We report a case of a huge cystic lipogranuloma of the retroperitoneal cavity in a 64-year-old man. The only clinical symptoms consisted of loss of appetite, increase of the abdominal girth and painful palpation in the left upper abdomen. Computed tomography of the abdomen revealed a large cystic mass, mimicking a malignant process in the retroperitoneum without relation to an intra-abdominal organ. The diagnosis of cystic lipogranulomatosis was made by laparotomy and histological examination. Whereas the etiology of lipogranulomatosis usually remains obscure, we have good evidence that in our patient a traumatic event, i.e. a blunt injury by a sword hilt, initiated the cystic lipogranuloma.
