Small intestinal biopsies in idiopathic steatorrhoea, intestinal lipodystrophy (Whipple’s disease), intestinal lactase deficiency and of fournormal volunteers wereanalysed by cytochemical and biochemical methods. The intestinal epithelium of idiopathic steatorrhoea showed in comparison to intestinal lipodystrophy, including lactase deficiency and healthy volunteers only little distortion of the cytoarchitecture of the cellular organelles visualized by the marker enzymes. The activity of the enzymes of the brush border however was reduced in many epithelia, the thiamine pyrophosphatase activity appeared more intense. Idiopathic steatorrhoea as well as intestinal lipodystrophy showed a reduction of the enzymatic activities for α-glycerophosphate dehydrogenase and β-hydroxyacyl-CoA-dehydrogenase by approximately 10-1. Contrary to idiopathic steatorrhoea, a normalization of these enzyme activities was found in antibiotica mediated clinical remission of intestinal lipodystrophy. The significance of the biochemical findings, equal to both lesions is discussed.
