Die Diagnose akute Appendizitis basiert auf Anamnese und klinischem Befund. Der lokalisierte und reproduzierbare Druckschmerz im rechten Unterbauch stellt das wichtigste diagnostische Kriterium dar. Unter den nichtinvasiven adjuvanten diagnostischen Verfahren hat die Sonographie den höchsten Stellenwert erlangt. Unerläβlich ist sie dann, wenn sie die Entscheidungsfindung im Rahmen der Differentialdiagnose zu beschleunigen vermag. Weitere differentialdiagnostische Maβnahmen haben sich dem klinischen Verlauf unterzuordnen. Die Laparoskopie ist die sicherste Methode bei der Klärung der Differentialdiagnose.
