Bis 1974 war die abdominoperineale Rektumexstirpation die Behandlungsmethode der Wahl bei Patienten mit Analkarzinomen. Die Strahlentherapie wurde nur bei inoperablen Patienten unter palhativen Gesichtspunkten eingesetzt. Französische Autoren publizierten jedoch der Chirurgie vergleichbare Behandlungsergebnisse mit primärer Strahlentherapie bei kleineren Tumoren des Analkanals. Dabei konnte bei etwa der Hälfte der Patienten die Sphinkterfunktion erhalten werden. Nigro veröffentlichte 1974 Ergebnisse einer Behandlung mit neoadjuvanter Radiochemotherapie und fand zum Teil komplette Tumorrückbildungen im Operationspräparat. Diese Erfahrung führte vielerorts zu weitergehenden Untersuchungen zur organerhaltenden Therapie dieses seltenen Tumors. Dabei wurden die beiden Behandlungsmodalitäten unterschiedlich dosiert und kombiniert. Die Behandlungsergebnisse waren vielversprechend, jedoch nicht miteinander vergleichbar und auf historische Kontrollen bezogen. Drei kürzlich publizierte randomisierte Studien der RTOG/ECOC, der EORTC und des UKCCCR definieren jetzt den Standard der multimodalen Therapie des Analkarzinoms: Radiotherapie mit simultaner Chemotherapie, wobei die Substanzen Fluorouracil als kontinuierliche Infusion und Mitomycin eingesetzt werden. Auch wenn in ausgewählten Fallen eine kurative abdominoperineale Rektumexstirpation bei fortgeschrittenen Tumoren noch möglich ist, so ist doch das Ziel der organerhaltenden Therapie in diesen Fallen verfehlt. Zur Zeit wird auf verschiedenen Wegen versucht, durch Modifikationen der Radiochemotherapie eine bessere lokale Tumorkontrolle bei diesen Patienten zu erreichen.
