Die operative Technik der Pankreastransplantation war lange Zeit unbefriedigend. Seit Verwendung von Pankreasduodenaltransplantaten und Ableitung des exokrinen Pankreassekretes in die Harnblase (Blasendrainagetechnik) ist die Pankreastransplantation aus chirurgischer Sicht sehr sicher geworden. Die Rate an «technisch» bedingten Transplantatverlusten und die perioperative Morbidität ist an spezialisierten Zentren dramatisch zurückgegangen. Die eigene, in einer persönlichen konsekutiven Serie von 83 Pankreasduodenaltransplantationen entwickelte Technik wird dargestellt und begründet. Mit dieser Technik konnte eine 1-Jahres-Funktionsrate von 85% erzielt werden. Künftige Entwicklungen im Bereich der chirurgischen Technik werden eine physiologischere Ableitung sowohl der exokrinen als auch der endokrinen Sekrete des Transplantates zum Ziel haben.
